What is a Category 3 Rail Medical?

The National Standard for Health Assessment of Rail Safety Workers places every rail safety worker into a risk category based on how directly their health could affect the safety of the rail network and the public.

The Cat 3 medical to Around the Track Personnel (ATTP) – workers who need access to the rail corridor but whose role isn’t safety critical. They’re responsible for their own safety and the safety of those working around them.

The assessment is focused on a clear question: can this worker detect and react to an oncoming train, move safely through the corridor, and work under supervision without putting themselves or others at risk? That’s what a Category 3 medical is designed to confirm.

The current edition of the National Standard came into effect on 11 November 2024.

How Cat 3 Rail Medicals Compare to Category 1 and 2

The three rail safety worker categories reflect the level of risk a worker’s health poses to the network and the public. Category determines both what gets assessed and how often.

Category 1 and 2 require a medical practitioner. Cat 3 medicals can be conducted by an AHPRA-registered health professional – including registered nurses, occupational therapists, and physiotherapists – provided they hold authorisation under the National Standard.

What’s Assessed: Category 3 Rail Medical Requirements

The Category 3 Track Safety Health Assessment focuses on conditions that could affect a worker’s ability to detect and respond to hazards in the rail corridor. It’s more targeted than Category 1 or 2 – but it’s not a formality.

Health questionnaire: Captures medical history, current conditions, medications, and any factors relevant to safe track access. Completed by the worker and reviewed by the assessing health professional
Vision assessment: Confirms the worker can identify hazards, read signals, and detect approaching trains at the required distances
Hearing assessment: Verifies the worker can hear auditory warnings, approaching trains, and instructions from supervisors
Mobility and functional assessment: Checks the worker can move safely in the rail corridor, including stepping clear of the track within the required time
Drug and alcohol screen: Required at pre-placement and on change of risk category. Not part of routine periodic re-assessment unless triggered

The assessing health professional documents findings on the National Standard’s prescribed forms and provides the Rail Transport Operator with a completed Health Assessment Request and Report Form.

Who Needs a Category 3 Rail Medical?

Any worker who accesses the rail corridor in a non-safety-critical role needs a current Category 3 medical assessment before they go near the track. Rail Transport Operators are responsible for identifying which workers fall within scope and ensuring assessments are in place before deployment.

Common roles requiring a Category 3

Track maintenance and civil works personnel
Vegetation management and environmental contractors
Utility and services workers operating within the corridor
Survey, inspection, and monitoring personnel
Construction and infrastructure contractors working on or adjacent to the rail network
Any worker whose role involves rail corridor access, even occasionally

When Re-assessment Is Required

Category 3 assessments aren’t a once-off. The National Standard specifies when workers need to be re-assessed.

Before commencement: Required before a worker accesses the rail corridor for the first time, or on first assignment to a role involving track access
Periodically from age 40: Every five years. Workers under 40 don’t require periodic reassessment under the current Standard unless a triggering event occurs
On change of risk category: Moving from Category 3 to a Category 1 or 2 role means a new assessment at the applicable category must be completed before the change takes effect
Triggered assessments: Required at any point if illness, injury, or a reported change in health status raises reasonable concern about a worker’s fitness for track access

For project-based and labour hire workforces, assessment currency is easy to lose track of when worker composition shifts between projects. Having a system in place matters – particularly when Rail Transport Operators ask for evidence of current assessment status.
