What Is a Commercial Drivers Medical and Who Needs One?

A commercial drivers medical is a health assessment for commercial vehicle drivers, conducted in line with the national Assessing Fitness to Drive (AFTD) standard published by Austroads. It assesses whether a driver is medically fit to hold a commercial vehicle licence or accreditation. You need one if you hold or are applying for any of the following:

Heavy vehicle licences: Medium Rigid (MR), Heavy Rigid (HR), Heavy Combination (HC), or Multiple Combination (MC)
Public passenger vehicle drivers: Bus, coach, taxi, rideshare (Uber, DiDi), chauffeur, and hire car drivers
Dangerous goods drivers: Any driver whose vehicle is required to carry dangerous goods
NHVAS accreditation: Drivers subject to Basic or Advanced Fatigue Management requirements under the National Heavy Vehicle Accreditation Scheme

The Commercial Vehicle Standard and Why It’s Stricter

Assessing Fitness to Drive sets out two distinct sets of medical standards: one for private vehicle drivers and one for commercial vehicle drivers. For every health condition covered in the standard, the threshold for a commercial licence is higher. This is deliberate. Commercial drivers spend more time on the road, operate larger and heavier vehicles, carry passengers or hazardous materials, and operate within the broader obligations of the National Heavy Vehicle Law, administered by the National Heavy Vehicle Regulator (NHVR). A health condition that qualifies a person for an unconditional private licence may result in a conditional commercial licence, or require further specialist review before any certificate can be issued. The current edition of Assessing Fitness to Drive came into effect in June 2022. All Safework Health commercial drivers medical assessments are conducted in accordance with this standard.

What the Assessment Covers

Every health assessment for commercial vehicle drivers covers the following areas:

Vision: Visual acuity and field of vision, assessed with and without corrective lenses. Commercial standards are more demanding than private licence requirements.
Cardiovascular health: Heart disease, arrhythmias, and hypertension are assessed against specific AFTD criteria for commercial drivers.
Diabetes: Blood glucose management, monitoring requirements, and the type of treatment in use all affect licence eligibility under the commercial standard.
Sleep apnoea and fatigue: Undiagnosed or inadequately treated obstructive sleep apnoea is a significant risk factor in heavy vehicle crashes. Drivers with this condition are generally required to demonstrate adequate treatment before a certificate is issued.
Neurological conditions: Epilepsy, stroke, and TIAs carry defined requirements around seizure-free periods and review intervals that are more stringent than private licence standards.
Musculoskeletal function: Adequate physical capacity to operate a heavy vehicle and respond in an emergency is assessed.
Hearing: Sufficient hearing to detect auditory warnings and communicate safely while driving.
Medications and medical history: All current medications and declared conditions are reviewed. Some medications have specific implications for fitness to drive under the commercial standard.

Conditions That May Require Specialist Review

Not every assessment results in a same-day certificate. Certain conditions require specialist reports, additional investigation, or referral to the Driver Licensing Authority before a commercial drivers medical certificate can be issued. These include:

Recent cardiac events
Insulin-managed diabetes
Moderate-to-severe sleep apnoea
Epilepsy
Certain neurological or psychiatric conditions

How Often Is a Commercial Drivers Medical Required?

Commercial driver health assessment intervals vary by state and territory, age, and health status. As a general guide:

Under 60: typically every 5 years
60–75: typically every 3 years
Over 75: typically every 1–2 years
Conditional licence holders: as specified in their licence conditions, often annually or more frequently

The NHVR and industry accreditation programs such as TruckSafe may require more frequent assessments independent of licence renewal cycles. Drivers also have a legal obligation to report any new or worsening health condition to the Driver Licensing Authority, not just at scheduled review intervals.

Previous medical certificate: If you have one from a prior assessment.
